City and state officials have closed an unlicensed care home on Moody Street after a 74-year-old man went missing from the property earlier this month.
The facility, operated by Hazel Forman, was not licensed to provide any form of elder care. According to state documents obtained by WFMY News 2, Forman admitted to helping residents with medication, bathing, and other personal care tasks despite lacking certification.
Police confirmed the home was officially shut down this afternoon. The action comes as part of an ongoing investigation connected to the disappearance of Larry Burton, who suffers from dementia. Burton was last seen at the location on July 3 and has yet to be found.

A nearby resident declined to be named and told ABC45 that closing the home was the right decision: “At this time, that would be the best thing for the neighbourhood until they find Mr. Burton and everything is figured out.”
Burton is described as approximately 5-foot-9 and 160 pounds, and was last seen wearing a teal polo shirt and blue jeans. The family offers a $5,000 reward for information leading to his safe return.
Burton’s grandson, Amare Swift, appealed to the public for help: “It’s genuinely not a fun feeling, and we want to bring him home if it’s possible.”
